[ARCHIVED] Chamber of Commerce Award Nominations - Due Oct. 12

The purpose of these awards is to honor and recognize outstanding community members, businesses and organizations that encourage and promote a positive and vital economy and preserve those unique qualities that are good for business and make the Cedar City area a very special place to live, work and visit.
Nominating Instructions: Select one of the categories below to nominate an individual, organization or business that has demonstrated significant achievement in 2012. Please read carefully the criteria for all award categories found on the next page. Describe in the space provided or on a separate sheet of paper how the nominee has distinguished themselves based on the criteria listed for the award category. Please provide specific examples. It’s also highly recommended that you attach supplemental items (newspaper clippings, award letters, etc.) with your nomination that demonstrates your nominee’s eligibility and worthiness. Please limit supplemental items to 4 pieces of 8 ½ x 11 paper.
DEADLINE: Award Nomination Deadline is October 12th. One nomination per form.
